Covid Protocols

Staff wearing mask as a covid protocol in front of Madison Family Dental Associates office building.Madison Family Dental Associates takes great pride in treating our patients and staff members safely during the COVID-19 pandemic by following guidelines from the Center for Disease Control (CDC) and the American Dental Association (ADA). At this time both of our clinic locations are open and are providing dental care services including preventive care and family dentistry, orthodontics, and periodontics.
We have implemented the following changes to help protect our patients, staff, and community.
•  Building access is limited and flow of patients through the clinic has been adjusted to minimize contact.
•  Daily health monitoring and temperature checks for all staff.
•  Prescreening health questionnaire for all patients and temperature checks upon entering the building.
•  To help protect our staff, we are using appropriate PPE and plexiglass dividers as needed.

As always, if you have any questions, please do not hesitate to contact your clinic. If you have any changes to your address, phone number, email, or insurance, please notify us prior to your appointment.

For the health and safety of patients and staff, you can expect to experience the following new check-in procedures:
•  Please come to your appointment alone unless you have special needs or are a parent/guardian attending with a minor patient.
•  If you are scheduled at our Madison clinic:
•  Call (608) 274-5970 when you arrive to check in for your appointment and remain in your vehicle.
If you arrive by bus, taxi, bicycle, on foot, or other means, or do not have a cell phone, please ring our doorbell and staff will let you in.

•  If you are scheduled at our Deforest clinic:
•  You may enter the building upon arrival. Given the lower volume of patients, we are able to accommodate social distancing in our waiting room

•  All patients entering the clinic are required to wear a face covering (mask, bandana, scarf, etc.)
•  Your provider will either call you when ready or will come to your vehicle and bring you into the clinic. Please have your mask on and be ready to enter the building.
•  Upon entering the clinic, your provider will take your temperature and promptly escort you to their room for your appointment.
•  Prior to starting your procedure, you will be asked to rinse with a hydrogen peroxide solution to help decrease potential viral load.
•  After your procedure is completed, we will ask you to reapply your face mask and exit the building.
•  Patients will be directed to leave the building from an exit other than the front entrance to minimize cross traffic.
